Home Health RN Case Manager

Amedisys Home Health of West Virginia

Amedisys is seeking a registered nurse case manager to support patients at home in Florida, coordinating care with the care team to ensure safe, effective treatment. You will assess patients, develop plans, supervise LPNs/HHAs, and communicate progress to the clinical manager.
