Registered Nurse - LTC/SNF

Nursing Position in Long Term Care/Intermediate Care Facility

Provide comprehensive nursing care to patients in a Long Term Care/Intermediate Care Facility, ensuring adherence to clinical standards and safety protocols. The role involves collaborating with interdisciplinary teams to support residents healthcare needs.

Responsibilities:

• Administer medications and treatments accurately according to physician orders.
• Monitor residents health status and document findings in the electronic health record system.
• Collaborate with healthcare team members to develop and update care plans.
• Respond promptly to resident needs and emergency situations.
• Maintain compliance with facility policies, regulations, and documentation standards.
• Float within the hospital and affiliate facilities as required, following Joint Commission standards.

Qualifications:

• Hold a valid state RN license or compact license in hand at the time of submission.
• Possess at least 1 year of relevant nursing experience.
• Demonstrate proficiency with PointClick Care and Cerner charting systems.
• Be flexible to work rotating 12-hour shifts, including day and night shifts.
• Be willing to work some holidays and floating assignments.
• Provide two professional references upon request.
