Plans, organizes, and manages the daily activities of RN Case Managers in accordance with SSLC and HHSC policies, procedures, state and federal rules, regulations, and ICF-IID standards. Sets goals and long‑range plans for SSLC RN case management services and provides clinical direction and expertise to RN Case Managers. Oversees performance indicators, audits, chart reviews, and the quality of the RN Case Managers’ participation in interdisciplinary teams. Essential Job Functions (EJFs) Attends work on a regular basis, may require specific shift schedule or rotating schedule, extended shift and overtime per agency leave policy. May serve in an on‑call status on a rotating schedule as required to meet operational needs of the facility. Sets goals, long‑range plans, and manages assigned personnel. Hires and assigns caseloads to RN Case Managers. Assesses and develops assignments to RN Case Managers based on patient needs and care complexity. Ensures caseloads are adequately distributed so individuals receive optimal case management services. Reassigns caseloads when necessary to balance resources. Establishes and evaluates nursing case management staffing plan identifying minimum resources needed to comply with regulatory and professional standards. Applies for staffing resources and reallocation needs to nursing management and takes action. Implements strategies to streamline work efforts and minimize redundancy. Tracks and aggregates information for feedback to program staff and leadership. Recruits and retains qualified personnel. Conducts performance evaluations and promotes awards and recognition activities. Provides clinical direction and expertise, ensuring nursing case management plans are based on appropriate assessment of resident needs. Oversees performance indicators to monitor health status and completion of tasks timely and with quality. Uses tracking system to oversee due dates. Conducts record reviews, audits quality work, provides feedback and corrective actions. Oversees interaction of RN Case Managers with individuals and teams. Conducts periodic rounds/observations of assigned RN Case Managers. Collaborates with Nurse Manager and other managers to ensure accurate information and quality services. Conducts monthly meetings to provide feedback and resolve issues. Oversees quality of participation in interdisciplinary teams and assists in resolving issues. Advocates for RN Case Manager perspective within IDT. Monitors nursing case management policies and procedures, recommends changes, and facilitates implementation of standards of practice consistent with Board of Nursing, ICF‑IID, CMS, TAC, and agency rules. Participates in quality assurance reviews and other activities to evaluate and improve nursing case management quality. Oversees implementation of local Quality Improvement plans (QIPs) and corrective action plans. Utilizes data to identify and address patterns and trends; oversees RN Case Managers’ analysis of data for incidents and issues such as emesis, falls, seizures, restraints, adverse drug reactions. Develops and updates training curricula, provides training to new and incumbent RN case managers, ensures required training is completed. Provides consultation, training, and technical assistance to nurses, IDT members and others. Facilitates coaching and supervises others in strategy development. Attends various meetings as assigned: morning medical, IDT, provider meetings, human rights, supervision, transition, pre‑placement, QA/QI, skin integrity/wound care. Completes required actions and documentation. Participates in other work groups: serious incidents, mortality review, federal monitoring, COOP activation, disaster response. Provides direct care nursing services and responses to urgent matters (e.g., high‑profile incidents). Administers medication, immunizations, treatment, observes patients for unusual symptoms. Provides treatment to HHS employees injured in course and scope of employment. Other duties as assigned, may require alternate shift pattern or location. Knowledge, Skills, and Abilities (KSAs) Knowledge of nursing techniques, standards, procedures; health care laws; medical diagnoses; and community health principles. Knowledge of state and federal laws and ICF‑IID rules, regulations, policies, and procedures related to State Supported Living Center. Excellent verbal and written communication skills. Skill in caring and treating patients. Computer and electronic health record system skills. Time management and prioritization. Strong interpersonal and team‑building skills. Strong coaching skills. Ability to interact with individuals with intellectual/developmental disabilities, families, and staff. Patient care, record maintenance, policy interpretation, recognition of medical necessity. Interpret regulations and communicate effectively. Organize, coordinate, evaluate nursing activities. Instruct, train, supervise others. Work independently and productively with all staff levels. Perform comprehensive physical assessments, identify abnormal findings, recognize signs and symptoms. Analyze, assimilate, integrate information to make nursing judgments, develop plans of care, evaluate outcomes. Provide nursing care in emergency situations. Maintain current CPR certification; strength and flexibility to safely perform CPR. Licensure, Certifications, and Education Registrant: Registered Nurse licensed in Texas or a state that recognizes reciprocity through the Nurse Licensure Compact. Initial Screening Criteria: At least 2 years of experience as a practicing registered nurse. Preferred: At least 3 years of experience as a practicing registered nurse in a health agency, hospital, or health‑care facility. Preferred: At least 2 years of RN Case Management experience. Preferred: Bachelor’s degree. Preferred: Experience working with IDD individuals. Males 18–25 must be registered with the Selective Service. All State Supported Living Center employees are subject to random drug testing. Flexibility in work hours may be required, including overtime, extended hours, weekends, and holidays. Must be at least 18 years of age to be considered for employment at a state‑operated facility.
