RN Case Manager

Medical City Healthcare is seeking an experienced RN Case Manager to coordinate patient care across the continuum in our Healthcare & Medical Services department. In this role, you will complete comprehensive assessments, develop individualized care plans, and collaborate with physicians, nurses, and ancillary teams to ensure safe, timely transitions of care. Responsibilities include discharge planning, utilization review, patient and family education, and connecting patients with community resources. You will monitor outcomes, support readmission reduction and quality initiatives, and maintain accurate EMR documentation. The ideal candidate is a clinically strong RN with solid communication, critical thinking, and case management experience.
