This role sits at the intersection of case management, discharge planning, and program coordination , ensuring seamless transitions for patients entering and exiting swing bed services while optimizing resource utilization and patient outcomes. Position Overview The Swing Bed Coordinator is responsible for managing the day-to-day operations of the swing bed program, serving as a key liaison between clinical teams, patients, families, and external providers. This individual will play a critical role in ensuring compliance, improving patient experience, and supporting the growth and efficiency of the program.
