Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families to accurately identify emotional, social, and environmental strengths and challenges related to their diagnosis or life circumstances.

• Develop and implement customized treatment plans utilizing evidence-based social work interventions that address patient needs effectively.
\n
• Collaborate closely with a multidisciplinary healthcare team—providers, nurses, and other specialists—to develop comprehensive care plans that involve patient and family participation.
\n
• Facilitate coordinated discharge planning, working with various stakeholders including consulting physicians, payers, and other healthcare entities to ensure a smooth transition post-hospitalization.
\n
• Provide education and counseling to patients and families regarding adaptation to illnesses or significant life changes, enhancing patients’ capacity for self-management.
\n
• Employ crisis intervention techniques, delivering both brief and long-term therapy, whether in individual, group, or family formats.
\n
• Act as a resource to other team members, providing guidance on social work best practices and psychoeducation.
\n
• Evaluate oncology patients for mental health challenges such as depression and anxiety, offering short-term supportive counseling as necessary.
\n
• Participate in on-call rotations to provide after-hours support for patients requiring immediate care.
\n
• When clinically indicated, facilitate involuntary examinations in accordance with the Baker Act provisions and coordinate with receiving facilities.
\n
• Represent the organization at educational conferences as a subject-matter expert in psychosocial oncology, broadening both personal and organizational influence in the field.
\n
• Engage in community outreach through educational initiatives designed to strengthen local resources available for cancer survivors and their families.

Patient Population / Client Focus\n

This position primarily serves a diverse patient population with unique psychosocial needs. Patients may come from various backgrounds and experiences, including those managing chronic illnesses, mental health disorders, or undergoing treatment for cancer. The LCSW will ensure that cultural competence is woven into every interaction.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid Licensed Clinical Social Worker (LCSW) license in Texas is mandatory.
\n
• Strong understanding of psychosocial theories and practices.
\n
• Exceptional communication and interpersonal skills to foster collaborative relationships with colleagues and clients.

Preferred Experience\n

• Experience providing psychosocial support in a healthcare setting, preferably with a focus on oncology or chronic illness management.

Compensation and Benefits\n

The position offers an annual salary range of USD 70,000 to 100,000, commensurate with experience and qualifications.
