Work Environment\n

This remote position allows for flexibility while working within a supportive team environment. As a member of our Hospice Center and early intervention services, you will collaborate with fellow professionals, including educators, therapists, and healthcare providers, to create cohesive support networks for our clients. Though primarily remote, you may periodically engage in on-site activities or meetings to foster team cohesion and enhance service delivery.

Key Responsibilities\n

In this role, you will:

• Conduct comprehensive evaluations and assessments to determine the needs of children and their families.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSP) tailored to each child's unique requirements.
\n
• Monitor progress and outcomes in collaboration with interdisciplinary teams, leveraging a data-driven approach to inform service modifications.
\n
• Educate and support families by sharing strategies and resources to facilitate their child's developmental progress in line with the Natural Learning Environment Practices.
\n
• Utilize evidence-based practices while fostering strong relationships with children, families, and team members to create a nurturing learning environment.
\n
• Ensure compliance with all federal and state regulations governing child welfare and social work practices.
\n
• Implement ongoing communication with families to provide insights and recommendations based on the child's development stages and family priorities.

Patient Population / Client Focus\n

The primary focus of this role is on infants and toddlers with developmental delays and disabilities. You will engage with children in their natural learning environments, facilitating enriching experiences that promote developmental milestones in physical, cognitive, social/emotional, and communication domains. Your interventions will empower families to become active participants in their child's life, enhancing the overall support network surrounding each child.

Qualifications\n

To be considered for this position, candidates should possess:

• A Master’s Degree or Doctorate in Social Work.

• Licensure as a Clinical Social Worker (LCSW) recognized by the Department of Public Health.

Preferred Experience\n

Compensation and Benefits\n
