POSITION DESCRIPTION

Title: PATIENT CARE COORDINATOR

General: Patient Coordinator works with referral sources, physician’s offices and patients to ensure patients receive their equipment and/or supplies. Patient Coordinators communicate with company billing personnel to provide documents needed for reimbursement.

Qualifications:  Graduate of an accredited high school or GED equivalency

Experience/Knowledge:  

1 year experience in DME (Durable Medical Equipment) businesses with a preferable knowledge in CGM (Continuous Glucose Monitoring) devices, Mobility equipment and breathing machines such as PAP devices and NIVs, . Brightree experience is Mandatory.

Good organizational skills. Detail oriented. Willing to function as a cooperative team member. Responsible and cooperative attitude. Clean and neat in personal appearance. Self-starter. Strong verbal communication and listening skills. High quality customer service. Product knowledge.  Brightree knowledge . Excellent attendance and dependability. Knowledge of insurance coverage, procedure codes, diagnosis codes and insurance fee schedules. Meet monthly daily and monthly productivity minimum.

Responsibilities and Duties:

• Patient demographics data entry
• Emergency Contact information
• Insurance details, verification and accurate plan assignment with appropriate pay scale
• Physician NPI, location details, contact numbers, PECOS verification
• Diagnosis appropriate to service provided
• Prior authorization requirements and submission
• CMN data entry, submission and logging
• Billing information for claims processing
• Sleep study, face to face notes including all required details for diagnosis
• Educate customer on insurance benefits, deductibles and co-pays
• Cleaning up pending orders & denials related to their assigned products.
• Scheduling

Completes electronic verification of insurance for DME coverage and prior authorization information, including but not limited to:

• Deductible
• Effective date
• Claims address
• Subscriber ID and group number
• Secondary insurance for part B coverage
