Job Description Summary MUSC Community Physicians (MCP) is an entity within MUSC that provides healthcare to patients throughout the state of South Carolina. Under clinical practice guidance of the Social Work Supervisor, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. Location and Schedule Location: Lexington County Detention Center. Day Shift: 9:00am – 5:30pm. Scheduled weekly hours: 36 hours. Pay type: hourly. Pay grade: Health-27. Responsibilities Assist with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. Complete assessment on the patient and caregivers throughout hospitalization and provide outpatient follow‑up as needed. Identify risk factors that the patient may experience after discharge and apply discharge interventions to lower the risk of undesirable events post‑discharge. Bridge the discharge to outpatient providers with the patient and caregiver to address issues and questions. Qualifications Master’s degree in social work or social welfare accredited by the Council on Social Work Education. MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
