Hospice Registered Nurse Case Manager

Park Nicollet is looking to hire a Registered Nurse to join our Hospice team! This position will be on the East Team (St. Louis Park, Edina Mpls etc). Position Summary The Hospice registered nurse assesses and evaluates patient care needs, provides skilled nursing care and teaching to patients and families in their home or community – based upon community standards of practice. The nurse is responsible to improve or maintain patient’s health status, provide terminal illness care, and to prepare patients and their families for self‑directed and/or assisted care in their community setting. The nurse coordinates medical, social, and financial resources to develop a comprehensive plan of care. The nurse supports the following community care processes: admission, discharge, billing, referrals, bereavement, and care delivery. Work Schedule 8-4:30pm M-F. Weekend call every 4th weekend. 2 holidays/year. Required Qualifications Education, Experience or Equivalent Combination: Baccalaureate degree in Nursing required. Work experience as a registered nurse for at least two years Licensure/ Registration/ Certification: Current license to practice as a professional nurse in Minnesota. Knowledge, Skills, and Abilities: Must be able to function independently with minimal supervision. High level of sensitivity, assessment skills, technical-skills, and communications skills. Demonstrates initiative, judgment, critical thinking, and the ability to problem solve. Demonstrates knowledge of age‑related growth and development principles necessary to provide appropriate service and assure safety of patients. Preferred Qualifications Education, Experience or Equivalent Combination: Experience in home care and or hospice desirable Must be able to function independently with minimal supervision. High level of sensitivity, assessment skills, technical-skills and communications skills. Demonstrates initiative, judgment, critical thinking, and the ability to problem solve.
