Job Title: Hospice Admissions Registered Nurse (RN) – Field-Based | Per Admission Pay

Company: Superior Hospice, part of the Home Health Care, Inc. family

Compensation: $200 – $250 per admission

Benefits: Full Benefits Package for Full-Time employees

Location: Greater Twin Cities Metro Area, 21-county service area (territories assigned based on coverage needs)

Schedule: Full-Time or Part-Time with 1 on-call shift per week (backup) and every 4th–7th weekend

What You’ll Do

• Travel to homes, assisted living, and care communities to complete hospice admissions.

• Perform eligibility assessments and initiate hospice services .

• Lead conversations around goals of care, expectations, and next steps .

• Build trust quickly with patients, families, and referral partners.

• Coordinate closely with intake, physicians, and the interdisciplinary team.

• Ensure a smooth, compassionate transition into hospice care .

• Complete accurate, timely EMR documentation .

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• Experience in hospice, oncology, palliative care, or hospital-based end-of-life care .

• Strong ability to lead conversations and build rapport quickly .

• Confident clinical assessment and decision-making skills.

• Comfortable working independently in a mobile, field-based role .

• Reliable transportation and ability to travel throughout the metro.
