Case Manager

Position Purpose:
This position requires the case manager primarily to meet with clients in the client's homes, but also requires visits to occur in schools and in the DC community.The ideal candidate would possess knowledge of evidence-based practices related to client engagement, trauma-informed case management, and service provision for both adults and children.In addition, candidates should be experienced in working with the following populations: low income, substance abusers, mentally ill, homeless, and the elderly.
Essential Duties and Responsibilities

• Uses evidence-based engagement strategies including assertive engagement, motivational interviewing, and other techniques that seek to develop meaningful helping relationships with clients.
• Develops written bio-psycho-social assessments to identify strengths and needs of families.
• Develops a service plan in collaboration with the family that identifies goals and tasks related to increasing self-sufficiency and well-being for both the parent and the child as well as the family as a whole; helps clients realize milestones and long-term goals.
• Identifies and links clients to community resources, transports clients to appointments and provides other supports in accessing those resources.
• Provides conflict resolution and therapeutic support to clients inclusive of helping to resolve ongoing family conflict and unresolved trauma.
• Participates in the enrollment of new clients into the program to include coordinating interviews, collaborating with partnering provides, assisting with identifying various move-in resources such as furniture, enrolling children into school, etc.
• Conducts school visits to check in on the school performance.
• Documents all client interactions in Housing the Homeless Database with thorough case notes, case reports, and case management plans within time frames.
• Participates in case conferences, case reviews, and clinical supervision processes to ensure a high quality of services for PSH clients.
• Works collaboratively with the Program Manager and other staff to provide services to all clients.
• Assists the Program Manager with reporting and various administrative tasks.

Minumum Position Requirements:

• Bachelor's degree

• Two years case management experience post bachelor's degree

• Must possess strong interpersonal, verbal, and written communication skills

• Must possess an attitude compatible with the goals and purposes of The National Center for Children and Families

Physical Demands/Work Environment:

While performing the responsibilities of the job, the employee is required to talk and hear. The employee is often required to sit and use their hands and fingers, to handle and feel. The employee is occasionally required to stand, walk, reach with arms, stoop, kneel, or crouch. Vision abilities include close vision with a computer monitor. Salary: $58,000 - $66,000
