Work Environment\n

This position is fully remote, allowing for flexibility while ensuring that you maintain consistent communication with your clients and team. You will engage with clients from the comfort of their homes, utilizing telehealth tools and platforms that facilitate seamless interaction. Our technology-driven framework not only supports effective engagement but also enhances the accessibility of our services. The work schedule is Monday to Friday, from 9 AM to 5 PM, aligning with your clients’ needs and the operational hours of our hospice center.

Key Responsibilities\n

• Conduct thorough case assessments to determine clients' clinical needs and personal circumstances.

• Develop tailored treatment plans in collaboration with multidisciplinary team members, ensuring alignment with the overall goals of patient care.

• Provide ongoing counseling and support to individuals facing end-of-life concerns, while also offering guidance to their families.

• Facilitate access to community resources and support services, empowering clients and their families to navigate their circumstances effectively.

• Remain updated on the latest practices in clinical social work and hospice care to continually refine and enhance delivery of services.

• Document patient interactions, treatment plans, and progress notes accurately in accordance with regulatory standards.

Patient Population / Client Focus\n

In this role, you will work primarily with terminally ill patients and their families, addressing the complex emotional, psychological, and practical challenges that accompany serious illnesses. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in Virginia.

• Proven experience in clinical assessments and treatment planning, particularly in hospice or end-of-life care.

• Exceptional communication skills, both verbally and written, fostering strong collaborative relationships within a healthcare team.

• Demonstrated capacity for empathy, resilience, and proactive problem-solving in a clinical context.

Preferred Experience\n

• At least two years of clinical social work experience, preferably in a healthcare or hospice setting.

• Experience with telehealth platforms and electronic medical records (EMRs).

Compensation and Benefits\n
