Will be required to watch up to 2 units on the weekend and focus on discharge planning. Weekend coverage required every other weekend. Must be available for holiday coverage. Years of experience: 2 years of acute hospital case management experience required. Required charting system experience: Epic. Participate in development of treatment plans. Perform bio-psychosocial assessments. Work with multi-disciplinary teams to ensure continuity of care. Educate patients and caregivers to facilitate discharge. Assist patients with navigating the Department of Children and Families (DCF) process. Conduct benefit eligibility assessments. Supervise staff effectively and efficiently. Participate in care conference activities. Coordinate services to home care and equipment following discharge. Discharge planning is a crucial aspect of the role. Knowledge of drug/alcohol abuse treatment options is required. Ability to organize and facilitate family meetings. Understand the needs of patients experiencing homelessness and mental health crises. Commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year's Eve, New Year's Day). Two references covering one year within the last three years are required. All travelers are expected to float between various facilities. Refusal to float may result in termination of assignment. Required to use computerized documentation systems. Knowledge of cultural issues is beneficial. Support provided by interdisciplinary teams, including physical therapy and pharmacy. Responsibilities may include case management for a caseload of up to 25 patients. Previous charge experience is beneficial but not mandatory. Participation in emergency detention procedures and handling involuntary holds may be required. Documentation must reflect understanding of coverage criteria and patient responses.
