9028 MI - Care Coordinator (N)

Caregiver - Flourish Collection at Oakland by StoryPoint Group Position Summary As a Caregiver you are responsible for providing caring and personalized assistance for residents living in our community. Required Experience High School Diploma or GED preferred. Previous Healthcare, Geriatrics or experience working with the elderly population preferred. Certified Nurse Aide (CNA) or Medical Assistant (MA) Certification preferred. Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form. Occasionally lift and/or move up to 25 pounds. Accountabilities Timely & Accountable Attendance: Uphold work schedules, be ready to start work on time, remain on the job for the whole duration of shift and use time off sparingly. 1440 Care for residents: Understand and lead our 1440 care standards so you and your team ensure that each resident and family has their needs met. Communicate resident changes immediately: Provide timely and detailed information to your Wellness Leader regarding any changes to a resident’s needs or care. Responsible communication with families: Communicate messages tactfully and timely with resident families, ensuring the message has been received and correctly understood. Build strong community relationships: With Wellness and community peers, families, residents and Leaders. Responsibilities Actively support our culture – our pillars and 1440. Complete daily/weekly tasks as assigned by the Wellness Coordinator, and assist others with their assignments if needed. Provide compassionate and competent care for residents by performing the following services: Bathing, showering and grooming assistance Assist residents with dining Provide wheelchair and walking escorts Provide toileting and light incontinence care assistance Assist with morning and bedtime routines Perform light upkeep of resident rooms Answer resident call signals Medication assistance, personal care/ medication administration, Assisted Living, Memory Care Complete and document appropriate records, including pertinent information entered into log book. Work scheduled shift but also provide flexibility and be open to helping other shifts if needed. Maintain knowledge of all safety and emergency procedures. Be knowledgeable of and abide by all company policies and procedures. Communicate any concerns regarding resident care, treatment, or changes in behavior to the appropriate person. Attend community staff meetings, in‑service programs, safety courses, and other required meetings. Stay up to date and knowledgeable of all departmental communication. General Working Conditions This position involves standing for long periods, communicating effectively, sitting, standing, walking and using hands to handle equipment and tasks. The employee must read, write, and speak fluent English, reach with hands and arms, push/pull equipment frequently, and sometimes bend, kneel, or squat. Appropriate interaction with others is required. The noise level is moderate. Occasional travel to different locations may be required.
