TASC Care Manager — Behavioral Health & Recovery Support
Coastal Horizons Center, Inc.
Coastal Horizons Center is seeking a TASC Care Manager to support justice-involved individuals across Hertford, Northampton, and Gates Counties. You will administer psychosocial screenings and coordinate access to behavioral health, medical, educational, and vocational services. The role requires strong communication, organization, and ability to manage a caseload independently.
