What You'll Do

Evaluate patients and develop individualized occupational therapy care plans
Help patients improve independence with daily living activities and mobility
Assess strength, coordination, safety, adaptive equipment, and home environment needs
Provide therapeutic interventions that improve function and quality of life
Collaborate with interdisciplinary teams to coordinate exceptional patient care
Complete documentation and clinical records accurately and timely
Educate patients and families to support long-term success at home

Qualifications

Active Occupational Therapy License
1 year of Occupational Therapy experience preferred
CPR Certification
Valid Driver's License & Auto Insurance
HomeCare HomeBase experience preferred

Physical Requirements

Ability to stand, walk, kneel, squat, and move frequently throughout the day
Ability to lift, carry, push, or pull up to 100 lbs.
