Key Responsibilities

• Conduct thorough evaluations, including reviewing patient histories, performing specific assessments, and developing personalized treatment plans.
• Effectively communicate evaluation findings and treatment strategies to patients and their families, integrating their feedback in the goal-setting process.
• Demonstrate proficiency in executing treatment protocols tailored to the unique needs of each patient and evolving healthcare practices.
• Collaborate with interdisciplinary healthcare providers when necessary and promptly document any changes in patient conditions that may require adjustments to their care plan.
• Oversee discharge planning and ensure appropriate follow-up care, maintaining precise documentation of all patient interactions and treatment notes.

Patient Population / Client Focus

The Physical Therapist will work with a diverse range of patients who are experiencing various musculoskeletal conditions, post-surgical recovery, and rehabilitation needs. Qualifications
