Case Manager Extended Care Coord RN-PD

About This Role The Case Manager Extended Care Coordinator RN is responsible for overseeing and managing skilled patients in rehabilitation at contracted Skilled Nursing facilities. This role is ideal for those who excel in coordinating care and collaborating with multidisciplinary teams. Highlights Pay: $78 - $95/hour Location: Los Angeles, CA Facility: Skilled Nursing facilities Schedule: Full Time What You'll Do Develop individual care plans based on patient assessments and diagnostic tests. Monitor and evaluate patient progress, modifying treatment plans as necessary. Recommend additional levels of care and therapy when medically indicated. Monitor appropriateness of therapeutic and rehabilitative care. Implement strategies to ensure patient and caregiver compliance with care plans. Provide individualized patient and family education focusing on self-management. Conduct individual and team conferences to identify patient risk factors. Facilitate patient return to daily activities with appropriate referrals. Develop treatment program procedures and clinical guidelines with the healthcare team. Educate inpatient case managers about transfer appropriateness to Skilled Nursing facilities. Ensure safe and appropriate patient discharges. Order durable medical equipment as prescribed by physicians. Screen for expected length of stay using senior metrics. Educate inpatient case managers about admission protocols to Skilled Nursing facilities. Intervene in quality issues surrounding transfers. Notify members about co-payments associated with Skilled Nursing transfers. Communicate with physicians regarding patient progress and status. Issue reports on patient care. Participate in interdisciplinary case conferences and consultations. Consult with physicians on appropriate length of stay for skilled patients. Contribute to staff education through in-service presentations. Attend inpatient UM staff meetings as recommended. Evaluate quality and outcomes of diagnostic and therapeutic services.
