Case Manager Extended Care Coordinator RN

Kaiser Permanente seeks a Case Manager Extended Care Coordinator RN to support patients needing ongoing and post-acute services. In this role, you will complete comprehensive RN assessments, create individualized care and discharge plans, and coordinate services across hospitals, rehab, home health, and community programs. You will collaborate closely with physicians, therapists, and social workers, ensuring safe transitions of care, appropriate utilization of resources, and strong clinical outcomes. Using EHR and data tools, you will track progress, document care, and contribute to quality and compliance initiatives in a hybrid environment.
