Colorado Care Manager: Field Visits & Care Coordination

FreedomCare is seeking a Care Manager in Colorado to oversee home-care coordination for Medicaid population. You will conduct quarterly supervisory visits, collaborate with care teams, and ensure safe, compliant in-home care across a defined Colorado coverage area. Ideal candidates hold a Bachelor's degree or equivalent, 2+ years bedside caregiving, and 1+ year telephonic outreach or call-center experience.
