Individual Service Coordinator

Position Full‑Time Individual Service Coordinator (ISC). Day Programs on Long Island. Location: Coram – 217 Middle Country Road, Westbury; 475 Union Ave, Brentwood, NY; 555 Washington Ave. Schedule: Monday‑Friday, 9:00am–5:00pm. Responsibilities Manage a caseload of 50‑70 people with serious and complex needs (PWS). Input services and meeting minutes into Evero. Take notes in meetings. Communicate with challenging families/residences. Understand clinical needs of PWS. Act as liaison between families/residences and care managers. Maintain records in accordance with OPWDD regulations. Assist with audits. Enter and manage databases of PWS. Provide coverage to the people we support based on program needs. Qualifications BA preferred. Experience working with people with intellectual and developmental disabilities and advocating for their needs/desires. Exceptional interpersonal, problem‑solving, and organizational skills. Strong presentation and communication skills. Willingness to take initiative. Highly flexible personality. Ability to deal with challenging families/residences. Understanding of person‑centered philosophy. Knowledge of Evero. Ability to work in a team environment. Compensation $19.05 per hour.
