Inpatient Case Manager - NBMC 2 North (Full Time, Day)

At NorthBay Health, the Inpatient Case Manager is to assess, plan, implement, monitor and evaluate options and services to affect an appropriate, individualized plan for hospital patient care across the continuum of care. The Inpatient Case Manager will follow assigned patients working with team members and Social Services to prioritize and strategize each day’s activities. Using independent judgment and discretion, the patient must be assessed and reassessed as an ongoing process to address the patient’s broad spectrum of needs.

The Inpatient Case Manager utilizes clinical knowledge and competence, positive communication skills, problem solving and conflict resolution techniques, ability to effect change, strong skills in assessment, organization, and time management. The case management process requires a focus on critical thinking skills, customer service skills, setting appropriate goals and measuring outcomes to effectively ensure optimal patient outcomes with consideration to financial and healthplan resources. The Inpatient Case Manager at NorthBay Health must be able to demonstrate adherence to the department and system policies, procedures, quality assurance, guidelines and goals of the department and the organization.

Qualifications

Education :

• Graduate from an accredited school of professional registered nursing; BSN required (excluding employees grandfathered under the March 2014 letter).

Experience :

• Minimum of 2 years of acute clinical nursing experience preferred.
• Case Management experience strongly preferred.
• Relevant experience may be considered in lieu of a Master’s in Nursing with an emphasis in Case Management.

Licensure/Certification :

• Current California RN license required. Certified Case Manager (CCM) or Accredited Case Manager (ACM) preferred.

Skills:

• Basic computer proficiency, including word processing and keyboarding skills.
• Ability to enter and retrieve data from relevant computer systems.
• Knowledge of MCG, Cerner, Medicare rules and regulations, and Conditions of Participation.
• Knowledge of Joint Commission standards, Department of Human Services requirements, Patient Safety initiatives, and Department of Health organizational standards.
• Understanding of levels of care, including acute care, home care, subacute care, long-term care, hospice, rehabilitation, and community resources.
• Familiarity with medical necessity guidelines, quality assurance processes, healthcare trends, best practices, management tools, and related resources.

• Adheres to established policies and procedures and meets attendance and punctuality expectations.

Physical Effort :

• Requires walking, stooping, bending, twisting, sitting, standing, and frequent computer use throughout the workday. Reasonable accommodations may be made for qualified individuals with disabilities.

Hours of Work :

• As assigned by the IPCM Supervisor and based on business needs.

Compensation:

• Hourly Salary Range: $80.73 - $97.33 (Offered hourly rate based on years of experience as a Case Manager)

Compensation Structure

NorthBay Health uses a structured compensation framework. Staff-level positions use a step-based system (Steps 1–5) based on years of directly related experience, with Step 5 representing 20 or more years of experience in the role. Manager level and above positions are paid a fixed annual base salary and are eligible for a variable incentive compensation plan. Physician compensation is structured based on specialty and role requirements.
