RN - Case Manager

Role Overview Responsible for assessing, developing, implementing, and evaluating patient discharge planning needs in collaboration with an interdisciplinary team. Ensures compliance with discharge planning standards, identifies barriers to patient care, and manages high-risk patient assessments to reduce readmissions. May oversee utilization management, emergency department case management, and act as a liaison with post-acute providers. Applies evidence-based practices and adheres to professional nursing standards, including the ANA Code of Ethics. Qualifications & Skills Graduate of an accredited nursing program Minimum of three years' acute care experience Valid RN license in applicable states (Illinois, Missouri, Oklahoma, Wisconsin) Physical ability to perform job duties, including lifting and mobility tasks Strong assessment, collaboration, and communication skills Key Duties Assess discharge needs and develop care plans Identify and elevate care barriers Collaborate with multidisciplinary teams Participate in quality improvement initiatives Perform related duties as assigned #J-18808-Ljbffr
