Job Description A medical facility located near Louisville, KY is seeking a full-time Director of Case Management to join its team. Pay: $100, 000 - $120, 000/Year Director of Case Management Opportunity:
- Full-time permanent position
- Director of Case Management manage the daily operations and HR functions of the Case Management department
- Directs the interdisciplinary care, discharge planning, and patient outcomes while championing patient advocacy
- Guides and trains staff, builds relationships with healthcare and insurance networks, and fosters a positive, celebratory work culture
- Current CCM or ACM certification or eligibility to obtain within first year of employment
- Must possess a bachelor's degree;graduate degree is preferred
- Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience
