Clinical Social Worker (MSW or LCSW): Home Health

We are a large home health agency providing care across the Bay Area and surrounding counties. We are currently hiring a Clinical Social Worker (LCSW or MSW) to work in the Placerville area. The position is per diem, with approximately 15-20+ visits per week. Responsibilities Assist patients and their families/caregivers with education and support as they cope with a recent diagnosis. Assess social and emotional factors related to the patient's illness and medical and nursing requirements in the home. Develop and implement a plan for home social work services, including education of patients, family members and caregivers. Coordinate with other home health staff and community resources to integrate care efficiently. Maintain harmonious, constructive relationships with internal and external customers (patients, families, interdisciplinary team, clerical and management staff, physicians, and community resources). Maintain and manage licensure and certifications required for the role. Plan and organize work, prioritize tasks, think critically, and resolve unique problems as they arise. Apply knowledge of local community resources, state and federal home‑care regulatory guidelines, and cultural influences on health care. Demonstrate effective written and verbal communication skills. Maintain a positive and professional image and pursue ongoing professional development. Qualifications Master’s degree from a program accredited by the Council on Social Work Education. Basic Life Support (BLS) certification. Driver’s license and automobile insurance. LCSW license highly preferred; supervision provided for license‑eligible Social Workers. One year of experience as an MSW in a health‑care setting, especially home health, strongly preferred. Knowledge of local community resources and understanding of individual development and human behavior related to illness/injury. Proficiency in cultural factors influencing health care. Strong written and verbal communication skills and ability to recognize the needs and concerns of diverse groups.
