Clinical Pharmacist Providence St Vincent Medical Center in Portland, OR – Per Diem/Variable Shift. Responsibilities The Clinical Pharmacist is responsible for providing high-quality, individualized, cost‑effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient medication therapy outcomes and quality of life. The final placement within this range will be based on relevant experience and other applicable factors.
