Registered Nurse MDS Coordinator

Care Village at Parkway Care Village at Parkway, a skilled nursing facility in West Roxbury, MA, is currently looking for a 32-hour per week RN MDS Coordinator with a special interest in care planning to join their team. Duties include but are not limited to: follow the MDS calendar to schedule resident care conferences complete comprehensive care plans in PCC in a timely manner utilize information from the medical record resident and staff interviews as well as quarterly assessments, consultant reports, etc. coordinate care plan meetings with interdisciplinary team, resident and resident representative update existing care plans as needed Requirements Graduate of an accredited school of nursing with current LPN/RN licensure in Massachusetts One year of Long-term care experience Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resources utilization, and/or case management is highly desirable Job Type: Full-time #J-18808-Ljbffr Care Village at Parkway
