Key Responsibilities

• Conduct thorough health assessments, which include taking detailed health histories and performing comprehensive physical examinations to evaluate patients' conditions.
• Develop and implement individualized treatment plans based on assessment results, ensuring alignment with the residents' healthcare goals.
• Prescribe and manage medications and therapies, closely monitoring patient responses and adjusting treatment as necessary.
• Interpret and analyze laboratory results, x-rays, and other diagnostic tests to inform clinical decisions.
• Engage in regular evaluations of patients’ conditions to determine the need for further healthcare interventions or referral to specialists.
• Document all patient care activities and educational interactions in detail within the electronic health record (EHR) system to ensure compliance with regulatory standards and continuity of care.
• Provide education to patients and their families on managing health conditions, medications, and preventive care, fostering a supportive and empowering atmosphere.
• Maintain up-to-date knowledge of medical best practices, participate in continuing education, and uphold all necessary certifications and licenses.

Patient Population / Client Focus

As a Nurse Practitioner at Medix Health Care, you will primarily work with the elderly population residing in our assisted living facility. This demographic often presents with a variety of chronic conditions, mobility issues, and age-related health concerns. You will play a crucial role in advocating for their health, addressing both their physical and emotional needs, and facilitating a care continuum that enhances their quality of life.

Qualifications

• Master’s degree in nursing from an accredited nursing program.
• Current Registered Nurse (RN) license in the State of Maryland.
• Certification as a Nurse Practitioner (NP) and a valid prescriptive authority.
• Demonstrated ability to work autonomously as well as collaboratively within a healthcare team.
• Excellent communication and interpersonal skills to engage effectively with patients, families, and staff.

Preferred Experience

• At least two years of clinical experience as a Nurse Practitioner, preferably in geriatric care or an assisted living setting.
• Familiarity with electronic health record (EHR) systems and other relevant healthcare technologies.
• Experience in developing patient education programs and participating in quality improvement initiatives.

Compensation and Benefits

We offer a competitive salary ranging from $150,000 to $160,000 annually, based on your level of experience and qualifications. Additionally, our benefits package includes:
