Key Responsibilities

• Conduct thorough physical assessments of patients to identify their unique needs and challenges.
• Design and implement individually tailored therapy programs aimed at improving mobility, reducing pain, enhancing balance, and addressing functional limitations.
• Administer hands-on treatments, including therapeutic exercises, gait training, and fall prevention strategies, ensuring a safe and supportive approach.
• Guide patients in regaining and maintaining autonomy over daily activities, fostering confidence and independence.
• Educate patients and their families about therapeutic exercises, wellness techniques, and care strategies to support recovery.
• Maintain comprehensive and accurate records of patient progress and treatment plans, complying with healthcare regulations to ensure continuity of care.
• Collaborate with multidisciplinary teams to ensure integrated care plans, monitoring patient progress and adapting therapies as needed.
• Report significant changes in patient conditions to the appropriate healthcare providers and family members to facilitate timely interventions.

Patient Population / Client Focus

This position primarily serves a diverse patient population, including geriatric individuals and those with chronic conditions requiring rehabilitation after surgery, injury, or illness. Qualifications

• An active Physical Therapist license in New Jersey is required.
• A Bachelor's Degree in Physical Therapy is essential, with a Doctorate or Master's Degree preferred.
• A minimum of 2 years of relevant experience in outpatient physical therapy is required, with preference given to those experienced in geriatric care or assisted living settings.
• Strong clinical skills and foundational knowledge in orthopedic and neurological rehabilitation methodologies.
• Exceptional communication abilities, demonstrating an empathetic approach to patient interactions.
• CPR and First Aid certification is preferred. Additional certifications in geriatrics or neurology are an advantage.
• A valid driver's license and reliable transportation may be required for any travel associated with patient home visits.

• Experience in developing and implementing evidence-based treatment plans in outpatient settings.
• Knowledge or experience with electronic health record (EHR) systems for maintaining patient records and monitoring progress.
