Responsibilities
- Provide comprehensive nutritional care to patients identified with nutritional risk or special needs
- Conduct patient assessment and reassessment, including nutrition-focused history and measurements
- Develop and update individualized nutrition plans of care; document interventions in accordance with policy
- Educate patients and families about nutrition therapy, dietary changes, and disease management
- Communicate effectively with the interdisciplinary care team to coordinate care and follow-up
- Apply clinical nutrition knowledge and decision-making skills to deliver high-quality patient care
