The incumbent assists with program oversight, monitoring, and administration, serving as a contact with local Public Housing Agencies (PHAs) and providing case management-based services (directly or through referral) necessary to ensure that veteran care is coordinated with the VA medical center, community health or mental health providers, and agencies that serve homeless populations. The incumbent works closely with the VA Health Care System's (HCS) homeless programs, residential and community care programs, and Domiciliary programs to ensure that coordination between these entities is facilitated and Veterans' direct care, referral, and follow-up is contiguous and based on a continuum of services. The incumbent independently provides clinical psychosocial and case management services at an independent practice level for eligible veterans and their family members in the program. The incumbent reports programmatically and administratively to the Supervisory Social Worker, HUD-VASH Program under the Domiciliary Service.

GS-09: All social work duties must be under the close supervision of a social work supervisor or licensed social worker of a higher grade.

GS-11: The work is reviewed for technical adequacy and conformance with procedures and practices applicable to the subject.

Duties include but are not limited to:

• Program Administration and Organization: The incumbent assists with data collection, administrative record keeping, and monitoring patient or program clinical indicators, thresholds, and performance measurements.
• Clinical Assessment and Diagnosis: The incumbent evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion, including an assessment of vulnerability and prioritization for admission.
• Referral, Rehabilitation, and Program Admission: The incumbent provides referral to VA medical and mental health inpatient and outpatient treatment and to other VA services, including VA benefits, as well as community-based social services and other non-VA entitlement programs.
• Comprehensive Case Management: The incumbent develops psychosocial treatment plans, actively involving the veteran and their family or significant others, in coordination with interdisciplinary team members, based on the psychosocial assessments, including goals for clinical treatment.
• Direct Services: The incumbent provides direct mental health and substance abuse services for veterans and family members or significant others in support of the veteran's treatment.
• Evaluation, Education, Supervision, Coaching, and Mentoring: The incumbent establishes a continuing relationship with the veteran, evaluating progress towards goals and adjusting the treatment plan as appropriate.
• Other duties as assigned

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).

Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.

Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.

Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
