- Assist with activities of daily living and support rehabilitation goals per care plans
- Collaborate with the charge nurse and interdisciplinary team to update care plans and communicate changes
- Document safety incidents and ensure adherence to infection control and safety protocols

Requirements
- Active LPN license in New Hampshire
- Graduation from an accredited practical nursing program
- Basic Life Support (BLS) certification
- Minimum 1 year of LPN experience in long-term care or skilled nursing settings
- Ability to work 3x12-hour night shifts and meet guaranteed-hour requirements
