CASE MANAGER II Full Time Professional Fairbanks, AK, US 1 Attachments 30+ days ago Requisition ID: 2515 Salary Range: $24.59 To $34.43 Hourly SUMMARY This position facilitates a seamless entry into services advocates through treatment and navigates complex service systems for consumers at the Ralph Perdue Center South , a 24/7 ASAM (American Society of Addiction Medicine) Level 3.1 clinicall y managed low-intensity residential treatment program serving adults with severe substance use disorders and co-occurring conditions. As these consumers may have faced significant stigma, trauma, and social, economic, and structural barriers to accessing treatment, this case management role is proficient in understanding substance use and assisting consumers in learning coping skills and creating stability in their lives ensuring a seamless transition through the full spectrum of care, bridging high-acuity stabilization and clinical progress as consumers move toward community reintegration and long-term recovery. JOB DUTIES Individualized Case Management & Clinical Support: Interview and screen consumers for case management needs; develops, individualized person-centered goals and objectives for the comprehensive treatment plan focusing on trauma-informed care, cultural competency, and sustainability. Coordinate consumer safety plans upon entry and develop robust transition/aftercare plans to ensure continuity of care throughout the recovery journey. Facilitates reimbursable skill-building groups using program approved evidence-based curricula and documents in the electronic health records according to standards that support medical necessity and audit compliance Maintain accurate, timely, and confidential electronic health record (EHR) and progress notes in compliance with 42 CFR Part 2 and HIPAA. Advocacy, Referral & Community Liaison Coordinates admission, transition and discharge logistics for consumers, managing travel arrangements from/return to remote communities, ensuring the collection of required documentation to meet ASAM, and other regulatory compliance standards for admission. Build and maintain an extensive network of community resources, including housing, transportation, education, medical and other healthcare, financial services, WIC, public assistance, legal resources and child protective services. Assist consumers with complex applications for WIC, public assistance, Medicaid, health insurance, workforce development programs, and other relevant applications. Function as a primary point of contact for the general public, referring agencies, and community organizations to provide program information and advocacy. Operational Coordination & Reporting Facilitate weekly interdisciplinary case staffing meetings between clinical staff and external systems of care to evaluate progress and refine individualized treatment plans, ensuring all coordination aligns with the consumer’s informed consent and self-determination Compile and submit monthly, quarterly, and annual reports to track program outcomes and compliance (as assigned). Ensure all services and documentation meet departmental policies, CARF regulations, and state/federal standards. Professionalism & Compliance Maintain active certification in CPR/First Aid and Non-Violent Crisis Intervention (MANDT or similar) on an annual basis. Demonstrate cooperative behavior and the ability to work under pressure, meeting multiple conflicting deadlines. Attend all required internal/external specialty trainings to stay current on trauma-informed and culturally relevant service models. Perform other duties as assigned. NECESSARY KNOWLEDGE, SKILLS, AND ABILITY Understanding of Substance Use Disorders (SUD) and co-occurring psychiatric conditions, including the application of ASAM Criteria for treatment and placement. Knowledge of how Adverse Childhood Experiences (ACEs) impact recovery, specifically regarding how parents in treatment may re-experience childhood trauma through their own children. Recognition of the vital role family, social networks, and community systems play in the long-term success of the treatment and recovery process. Ability to engage with interdisciplinary treatment teams to provide integrated care. Ability to provide non-judgmental and empathic care for pregnant and parenting consumers with substance use disorders. Proven skill in working successfully with Alaska Native/American Indian populations, with the ability to develop and provide culturally specific programming. Understanding of and sensitivity to diverse cultures, lifestyles, and non-stigmatizing language. Ability to assist and stabilize escalated individuals, particularly during crisis episodes or high-pressure clinical encounters. Ability to maintain consumer confidentiality, basic knowledge of HIPAA and 42 CFR Part 2. Skill in operating computers, including Windows and the Microsoft Office Suite (Word, Excel) for professional documentation and analysis. Above-average interpersonal, oral, and written communication skills; ability to follow complex instructions and deliver clear analytical reports. Demonstrated ability to manage multiple conflicting priorities and meet deadlines while maintaining strict punctuality and attendance. Proven skill in working both independently and as a collaborative team member to maintain cooperative relationships with colleagues, supervisors, and external agencies. MINIMUM QUALIFICATIONS (Education & Experience) Bachelor’s Degree in behavioral health, social services or related field. Two (2) years’ experience involving case management. One (1) years’ experience working with AN/AI population and experience providing culturally specific services.
