Responsibilities
- Plan and conduct segments of physical therapy treatment programs to restore and improve physical function and mobility for patients in a home-based setting.
- Assess patient goals, document progress, and adjust treatment plans to align with department objectives and patient needs.
- Serve as a co-leader in an interdisciplinary approach to patient care, collaborating with other clinicians to optimize outcomes.
- Educate patients and caregivers on safe and effective home exercise programs, transfers, gait training, and fall prevention.
- Monitor and document patient responses to therapy, ensuring timely charting and compliance with evidence-based guidelines.

Requirements
- Active physical therapy license in California (CA).
- Doctor of Physical Therapy (DPT) or equivalent degree.
- BLS certification required.
- Ability to work effectively in a home health/home-based rehab setting with patients and families.
- Willingness to travel for a 13-week assignment; valid driver’s license and reliable transportation.
- Strong communication skills and ability to collaborate with an interdisciplinary team.
- Prior home health, outpatient, or inpatient rehab experience preferred.
