In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) -Bellingham, WA

Signify Health, part of CVS Health®, is seeking a Part‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. Responsibilities Bring your heart into every visit, joining a national network of purpose‑driven clinicians dedicated to improving health outcomes. Visit millions of people nationwide, connecting them to the right care for their unique needs. Work as a clinician colleague with Signify Health, fulfilling the specific role requirements. Be flexible to travel locally and within licensed states as needed. Conduct virtual visits when required. Provide all Signify Health services as indicated, including diagnostic and preventive services such as spirometry, DEE, bone density screening, and others. Work Environment As an in‑home clinician, your primary workplace is the private residence of health plan members. You must be able to comfortably and safely perform your duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Qualifications NP or PA, active and unrestricted license(s) in the coverage area(s). Board certification required. Multi‑state license(s) preferred. Ability to travel locally and within licensed states. Flexibility to work part‑time hours (average 29.5 hours per week). Compensation Pay Range: $46.03 – $99.14 per hour. The pay range represents the base hourly rate for the In‑Home Health Evaluation and is subject to factors such as experience, education, geography, and other relevant criteria. Additional reimbursement may be available for completed Diagnostic and Preventive Services.
