- # LVN, Case Manager Liaison - Home HealthApply: Fort Worth, TX: Full time: Posted Today: JR-119762**Location:**Home Health - Fort Worth**Department:**Administration**Shift:**First Shift (United States of America)**Standard Weekly Hours:**40**Summary:**The LVN, Case Manager Liaison will coordinate efficient and effective transition for patients from hospital setting to home environment while maximizing patient outcomes. **Education:*** The formal education from an accredited college or university in area of nursing. Education and job training as a Licensed Vocational Nurse with current State of Texas licensure (or license eligible) is required.* Minimum of 3 years pediatric experience required; 2 years of home health and/or discharged facilitation/case management experience preferred.* Knowledge and understanding of pediatric home care.**Certification/Licensure:*** Current State of Texas licensure (or license eligible) is required.* Basic life support (BLS) certification or scheduled attendance of a CPR certification course.
