Responsibilities

• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents’ health status, observe for changes, and report promptly to the nursing team.
• Provide routine nursing care and support with activities of daily living as appropriate.
• Document care provided and update resident charts and care plans accurately and timely.
• Collaborate with RNs, charge nurses, and interdisciplinary team to ensure coordinated care.
• Maintain safety, infection control, and quality-of-care standards in daily practice.

Requirements

• Active Licensed Practical Nurse license to practice in New Mexico or eligibility to obtain it.
• Experience in a long-term care or geriatric setting preferred.
• Ability to work night shifts and participate in a travel assignment.
• Effective communication and documentation skills; detail-oriented and reliable in following care plans.
• Demonstrated teamwork and ability to collaborate with diverse healthcare staff.
