Responsibilities

• Provide direct patient care to residents in a long-term care setting under supervision of a registered nurse
• Administer prescribed medications and document administration according to policy and physician orders
• Monitor vital signs, observe resident status, and report changes promptly to the supervising nurse
• Assist residents with activities of daily living, mobility, hygiene, and feeding to support independence
• Maintain accurate charting, care plans, and shift handoffs to ensure continuity of care
• Uphold infection control practices, safety protocols, and regulatory compliance during all tasks

Requirements

• Active LPN license in good standing (standard qualification for the role)
• Ability to work effectively in a long-term care setting and manage physical duties
• Basic Life Support (BLS) certification
• Prior experience in long-term care or similar clinical settings preferred
• Strong communication and teamwork skills, with ability to document clearly and collaborate with the care team
