Integrated Case Manager: Insurance & Care Coordination

Hackensack Meridian Health seeks an Integrated Case Management Specialist to support insurance management, payer communications, and discharge processes. This role collaborates with Care Coordinators and uses EPIC to manage data and denials. You will coordinate Medicaid appointments, assist with discharge planning, and provide documentation for Medicare letters.
