Seven Healthcare is pleased to offer a per diem opportunity for an RN Adult Home Health in Raleigh, NC. This assignment consists of flexible day shifts ranging from 2-hour to 8-hour visits, providing skilled nursing care to adult patients in their homes. Compensation is $67.50 per hour.

Key Responsibilities of the RN Adult Home Health:

• Provide skilled nursing care to adult patients in a home setting according to individualized care plans
• Perform comprehensive patient assessments and monitor changes in health status
• Administer medications, wound care treatments, IV therapies, and other prescribed interventions
• Educate patients and caregivers on disease management, medication administration, and health maintenance
• Monitor chronic conditions such as diabetes, heart failure, COPD, and hypertension
• Coordinate care with physicians, therapists, case managers, and other healthcare professionals
• Maintain accurate and timely documentation of patient visits and care provided
• Promote patient independence, safety, and quality of life within the home environment

Requirements:

• Minimum 1 year of recent RN experience, preferably in home health, medical-surgical, telemetry, rehabilitation, or community health settings
• Active North Carolina RN license or Compact RN License
• Current BLS certification required
• Strong assessment, clinical decision-making, and patient education skills
• Experience with wound care, medication management, and chronic disease monitoring preferred
• Ability to work independently and effectively manage a flexible patient schedule
• Reliable transportation and willingness to travel within the service area
