Wellness Manager {RN or LPN)

Employment Type Full‑time Location Sonata Palm Coast, Palm Coast, FL Job Summary Responsible for carrying out the instructions of the Director of Wellness. Supervise and monitor nursing staff to obtain the highest quality care by performing routine nursing duties. Accountable to the Director of Wellness/Executive Director for all nursing department activities and maintaining the corporate philosophy of quality care. Duties and Responsibilities Supervise HHAs, CNAs and facility staff. Make daily rounds of all Assisted Living residents and retirement residents if needed. Observe and report symptoms and conditions of patients to the Director of Wellness or delegated RN/Physician. Administer medications and treatments per signed doctor’s orders in an accurate, safe manner; observe effectiveness and report to physician if needed. Inform the Director of Wellness of when supplies and equipment are needed. Order medication from pharmacy when needed. Supervise the Home Health Aide to ensure that all nursing care is given properly, safely and of the utmost quality. Administer emergency first aid. Ensure daily assignments for patient care, incidental duties performed by nurse and HHA as outlined by the Director of Wellness are carried out. (Documentation: vitals, weights, nurse notes, medication, kardex, etc.) Make daily rounds of all patients and area. Recognize, interpret and report conditions to the Director of Wellness/RN/Executive Director as needed. Maintain cleanliness and neatness of nurse’s station, med room and cart. Assist in orientation of new staff. Complete admission and discharge evaluations as necessary. Check equipment for cleanliness and condition. Provide guidance for nursing assistants. Promote and maintain good interpersonal relationships with residents, visitors and personnel. Perform related work as required per assignment; understand duties for assisted living and independent residents throughout the facility. Note The above duties include many of the tasks for LPN. At times it may be necessary to perform other duties as requested by the Assisted Living Manager. Refusal to perform any task as directed by the General Manager will be cause for disciplinary action. Qualifications Graduate of an accredited professional nursing program, currently licensed to practice per state. Work experience in the geriatric field. Ability to communicate well in writing and orally. Ability to deal with visitors, staff and residents in a courteous manner. Ability to lift, push, pull up to 60 lbs. Strong organizational skills. Must be able to work under pressure at times. Must perform good grooming practices, maintain personal hygiene and be punctual with regular attendance. Must have a warm, friendly, congenial personality; present a professional appearance and possess an empathetic attitude toward seniors. Ability to assess a resident’s condition and intervene when needed. CPR certified, or obtained within 90 days of hire and renewed yearly thereafter. May have their picture taken for use in social media or community advertising. May be video recorded from devices installed by families in residents’ apartments.
