Position Overview Licensed Practical / Vocational Nurse – LPN/LVN to provide skilled nursing care under the supervision of a Registered Nurse and in accordance with patient care plans. Location San Antonio, TX (ZIP: 78251, 78242, 78250, 78207, 78253, 78236 and surrounding areas). Responsibilities Deliver skilled nursing care and document care according to physician orders. Continuously assess patient condition and report changes to supervisor/physician. Document all nursing assessments, actions, and care coordination in a timely manner. Participate in updating nursing care plans. Perform nursing actions based on assessment to achieve expected outcomes. Identify changes requiring intervention and implement care to prevent risk. Comply with Aveanna policies, state and federal regulations, and the Nurse Practice Act. Use infection control measures per OSHA standards. Educate patients and families about disease processes, self‑care techniques, and prevention strategies. Maintain competency through continuing education and skill evaluations. Qualifications Graduate of an accredited school of nursing. Active, unrestricted LPN/LVN license in the state of practice. Current CPR certification. Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing policies. Reliable transportation. Physical ability to lift 50 pounds, reposition patients without assistance, and perform prolonged walking, standing, bending, kneeling, and reaching. Ability to travel and work in varied environments with potential exposure to allergens, blood, bodily fluids, and infectious diseases. Preferred Experience At least six (6) months recent experience as an LPN in a clinical setting. Home health experience. State‑specific requirements: South Carolina – one (1) year of pediatrics experience; California – one (1) year of nursing experience; Louisiana – one (1) year of licensed nursing experience. Physical Requirements Speak, write, read, and understand English. Travel to patient sites as needed. Lift 50 pounds and reposition patients without assistance. Prolonged walking, standing, bending, kneeling, reaching, and twisting. Climb stairs and maneuver in environments with exposure to allergens and infectious materials.
