Most clients receiving representative payee services have stable housing and a professional support team in place.Our Account Managers provide ongoing monitoring of wellbeing and care coordination to help clients maintain housing community stability. For individuals entering services due to homelessness, risk of homelessness, exploitation, eviction intervention, or other emergent circumstances, we provide outreach and community-based support.

The Case Manager works closely with management, account managers, and community partners to ensure individuals receive coordinated, compassionate, and effective services.

Minimum Requirements:

• This position requires a Bachelor’s degree in social work or related field and less than two (2) years of experience in the field; OR at least five (5) years of experience in the field with no degree.
• State police criminal record check for any state resided in during the previous year. Applicants with convictions listed in KRS Chapter 17.165 (1)-(3) or any prior felony conviction, cannot be employed.
• Demonstrated ability in needs assessment, accessing local resources, advocacy, counseling, and working cooperatively with service providers. Ability to work in a fast-paced environment.
• Valid state driver’s license, liability insurance, and reliable transportation.
• Ability to understand and commit to the Kept, Inc. mission and values.

Primary Responsibilities

• Our goal is to foster economic mobility, housing stability, and health equity for individuals facing homelessness, particularly.
• Advocate for clients and act as liaison with landlords, rental agencies, community organizations, and other service providers.
• Demonstrate cultural sensitivity and trauma‑informed communication when working with adults experiencing homelessness.
• Conduct client assessments, develop and monitor care plans, nd complete follow‑up activities while addressing dynamic needs, including mental health challenges and chronic homelessness.
• Coordinate with community partners to connect participants with essential healthcare, mental health, and recovery programs, alongside supportive services like financial planning, legal aid, transportation, and housing counseling.
• Assist clients in obtaining essential documentation through account manager collaboration (birth certificates, driver’s licenses, SSA cards, health cards, TARC tickets).
• Provide community‑based care coordination, including pickup/drop‑off of housing items for individuals transitioning into housing.
• Conduct intakes at VSD homeless sites and provide care coordination at homeless shelters.
• Maintain positive relationships and open communication with all Kept Inc staff and community partners.
• Complete administrative tasks and other dutiesas outlined in the performance metrics document.

Position Attributes:

This hybrid position requires strong scheduling coordination and flexibility during daytime business hours to complete tasks involving community partners and clients. This role is a hybrid schedule requiring 50% on-site in the Louisville community and 50% remote work from home. While performing duties of this job, the employee is frequently required to drive for short trips within Jefferson county, visit homeless shelters, attend social security appointments in-person, attend LIHEAP appointments in-person, obtain legal documents in-person, transport household materials to clients, etc. This is a full‑time, grant‑funded role with an hourly rate of $19.60 and a maximum annual compensation of $40,768 .
