You will provide ongoing education and emotional support to patients and their loved ones, ensuring they understand the plan of care, what to expect, and available resources. Documentation is completed in an electronic medical record system, and you will participate in regular team meetings, care conferences, and handoffs to ensure continuity and quality of care. Patient ratios are designed to support safe, attentive hospice practice, allowing you to spend meaningful time at the bedside.
