Masters Case Planner - Brooklyn, NY (Onsite) – Full Time – $27.00-$30.00 Per Hr Title: Speech Language Pathologist Location: Brooklyn, NY (Onsite) Employment Type: Full Time Compensation: $27.00-$30.00 Per Hr Job Summary This full-time Masters Case Planner position is based in Brooklyn, NY, and involves working with a team of healthcare professionals in the foster care program. The role focuses on providing casework services and fieldwork for children with special medical needs and their families, aiming for timely permanency. The planner will collaborate with medical staff and community resources to support children and families. Requirements Provide intensive casework services for children and their families. Promote the organization's mission and values. Collaborate with nurses and social workers in the foster care program. Conduct outreach to connect families with community resources. Maintain accurate and detailed written case records. Conduct home visits in partnership with assigned RN. Perform other duties as assigned. Qualifications Bachelor's Degree in Social Work or related field required. Foster care experience preferred. Bilingual (Spanish/English) preferred.
