Responsibilities
- Conduct initial and ongoing PT evaluations for residents in a geriatric, skilled nursing setting.
- Develop and implement individualized treatment plans focusing on mobility, strength, balance, gait, endurance, and activities of daily living.
- Reassess progress regularly, document outcomes, and adjust plans accordingly.
- Educate residents and families on exercise programs, safety strategies, and home programs to support continued recovery.
- Collaborate with the interprofessional team to coordinate care, discharge planning, and goals alignment; maintain accurate, HIPAA-compliant documentation.
- Ensure safe patient handling, adherence to infection control practices, and proper equipment maintenance.

Requirements
- Active Physical Therapist license in Washington state or eligible for licensure in WA
- Ability to practice in a skilled nursing facility / geriatric rehabilitation setting
- Strong communication and teamwork skills to coordinate with a multidisciplinary care team
- Proficiency with clinical documentation and electronic medical records
- Availability to fulfill the scheduled shift and maintain 35 hours per week
