Job Title Physical Therapist Location Colorado Schedule / Job Type Contract | Days | Outpatient Setting | 6-Month Contract Key Responsibilities Conduct comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities Delegate and direct all discipline-specific therapy services for assigned patients Serve as an educator to patients, families, students, and coworkers; participate in student programs as assigned with appropriate supervision and evaluation Adhere to discipline-specific State Practice Act and all regulatory requirements; maintain professional licensure in good standing and complete all state-mandated continuing education requirements Required Qualifications Active and unrestricted Physical Therapist license in the state of Colorado Ability to perform 60-minute evaluations and 30-minute follow-up appointments in an outpatient setting Proficiency in clinical reasoning, treatment planning, and goal writing aligned with functional outcomes Knowledge of and adherence to discipline-specific professional codes of conduct and ethical standards Ability to ensure care is provided under signed, valid physician orders and within insurance approvals Preferred Experience / Skills Prior experience in an outpatient physical therapy setting Experience supervising or mentoring physical therapy students Familiarity with performance improvement methodologies and organizational quality initiatives Why Join Us Opportunity to work in a structured outpatient environment with a focus on patient-centered care and functional outcomes Supportive setting that encourages ongoing professional development and continuing education Concentric Healthcare Staffing Job ID 769475.
