Physical Therapist (PT), Home Health, PRN, 1099

Essential Functions & Responsibilities of the Physical Therapist (PT):

Perform comprehensive assessments and evaluations of patient care needs, including: functional status and mobility, endurance and stamina, pain levels and other medical conditions

Develop and implement individualized treatment plans as a Physical Therapist (PT)

Provide skilled interventions to improve patient well-being and functional outcomes

Monitor and evaluate patient progress, adjusting care plans as needed

Deliver thorough education and training to patients and caregivers

Participate in staff and administrative meetings

Qualifications for the Physical Therapist (PT)

Active license from the Florida Board of Physical Therapy

Valid Florida driver’s license, active vehicle insurance, and reliable transportation

Ability to pass AHCA Clearinghouse background screening (

Current in-person CPR certification

Physical ability to perform job duties including standing, bending, lifting, and assisting patients/equipment

Strong customer service, communication skills and effective listening

Efficient and timely documentation

Experience Preferred for the Physical Therapist (PT)

Minimum 1 year of Physical Therapy experience

Minimum 1 year of Home Health experience

Compensation for the Physical Therapist (PT)

Pay is commensurate with Physical Therapist (PT) experience, Home Health background, and documentation proficiency
