Overview
Physical Therapist – Home Health in Arcata, CA, starting 06/22/2026 for 13 weeks. Weekly pay: $2,180–$2,364 per week with guaranteed 40 hours. Shift: 5x8 Days. Travel - temporary assignment in a home health care setting, focused on delivering skilled PT services to patients in their homes.

Responsibilities
- Conduct initial and ongoing functional assessments of home-based patients and develop individualized PT plans of care.
- Implement evidence-based PT interventions to improve mobility, strength, balance, gait, and pain management.
- Regularly reassess patient progress and modify the plan of care to meet goals and optimize outcomes.
- Educate patients and caregivers on exercises, activity modification, safety, and use of assistive devices; document home exercise programs.
- Maintain accurate documentation in the electronic health record and communicate findings with the supervising PT and the care team.
- Ensure patient safety and infection control in home environments and support appropriate discharge planning.

Requirements
- Active California physical therapist license
- Basic Life Support (BLS) certification required
- TB clearance completed within the previous 12 months
- Ability to work in a home health care setting and travel to patient homes as needed
